How to use this sheet
- Print the PDF — one page covers about three weeks of daily entries.
- Each day, rate your overall pain severity from 0–10, even on good days. A zero is valuable data.
- Note the primary location and pain type — these often change and the change is clinically meaningful.
- Record anything that seemed to make pain worse: activity, weather, poor sleep, stress, or certain movements.
- Note what helped: rest, heat, ice, medication, physical therapy. In the final column, briefly rate how pain limited your function.

Should I log every day even when pain is low?
Yes — and especially on good days. Low-pain days often share identifiable factors (good sleep, reduced stress, certain activities avoided) that are as clinically valuable as the high-pain days. A complete log is far more informative than one that only records bad days.
Can I use this for fibromyalgia, CRPS, or back pain?
This sheet is designed to be condition-agnostic — it tracks the dimensions of pain that are relevant across conditions. The location, type, and aggravating-factor columns can be adapted with whatever terminology is relevant to your diagnosis.
