How to use this sheet

  1. Print the PDF and post it near where you store the medications.
  2. In the top section, list every current medication: name, dose, how often it's given, and when the supply runs out.
  3. Each time you give a dose, fill a row in the log section: who gave it and when — this prevents accidental double-dosing when two people share pet care.
  4. Check the refill dates weekly — running out of a heart or thyroid medication is a genuine emergency.
  5. Bring the sheet to every vet visit and any emergency clinic visit.
