How to use this sheet
- Print the PDF — one page covers about three weeks.
- Each evening, rate your overall gut severity from 0–10 and check the symptoms that were present: bloating, cramps, urgency, nausea.
- Record how many bowel movements you had and the approximate Bristol type for the predominant one.
- Note anything that might explain the day — a stressful event, poor sleep, a new food, a missed medication dose.
- After two to three weeks, look for the pattern: the same factor appearing before your worse days. Bring the log to your appointment.
