How to use this sheet
- Print the PDF — one page covers about three weeks.
- Log each day's meals, circling or noting any high-FODMAP foods — onion, garlic, wheat, dairy, and certain fruits are common examples.
- Record your bowel type using the Bristol Stool Scale (1–7) and rate bloating and pain from 0–10.
- Note your stress level — IBS and stress are closely linked, and stress is often overlooked in food-only diaries.
- After two or three weeks, bring the filled sheets to your gastroenterologist or dietitian.
