How to use this sheet

  1. Print the PDF — use one page per week during the elimination or reintroduction phase.
  2. For each meal, list what you ate and estimate the FODMAP load: low (safe-list items), medium (borderline), or high (known high-FODMAP).
  3. In the Symptoms column, note any gut reaction that appeared in the two to four hours following the meal.
  4. During reintroduction, test one FODMAP group at a time over three days — the log makes it clear which days triggered reactions.
  5. Bring the completed log to your dietitian; it provides the data needed to confirm your personal FODMAP tolerances.

What is the low FODMAP diet?

FODMAP stands for fermentable oligosaccharides, disaccharides, monosaccharides, and polyols — types of carbohydrates that can trigger IBS symptoms in sensitive individuals. Do I need to know exact FODMAP amounts?
